Those questions are addressed in the new preview for this week's Vampire Diaries episode. The video begins with a clip of Damon asking Alaric if he's with "the sexy psycho doctor." Alaric just hangs up, but he is. He tells Meredith that Damon's "a little judgmental" when she asks if he still thinks she's the killer. That's putting it lightly, but really, Damon has a point. Just because she came clean about using vampire blood to help her patients doesn't mean she's not hiding something else—like killing people, too. After all, it was her ex who ended up dead soon after calling her crazy. Nina Dobrev explains that no one has come forward taking responsibility for these deaths, which is strange for this town. Who's staking and stabbing people using their weapons?
Meanwhile, Bonnie and Abby are going to be meeting with Esther, who calls them "allies," and even though she's planning to kill all her children just to kill Klaus, it's hard to not wonder if she's hiding something from Elena (and possibly Finn, who's helping her). It can't be that easy to get rid of Klaus, can it?
Nina Dobrev also compares each Original, and clips show Rebekah with Kol and Rebekah telling Elijah their mother's back "to make [their] family whole" and nothing else. She doesn't think Elijah's going to find anything. Oh, if she only knew the truth as she says that. Rebekah's obviously delighted to have her mother back, so how will she react when she finds out the truth?
Finally, there's the issue of Elijah's loyalty on The Vampire Diaries. As Ian Somerhalder puts it, "You never really know who he's rooting for." He has betrayed them before, but with it being "every man for himself," anything's possible. You don't want to see Elijah die, but at the same time, it doesn't look like he's reacting well in this episode, as he does tell the Salvatore brothers they have to help him stop his mother if they want to save Elena. Will Damon and Stefan decide to help him stop Esther from doing the one thing they know can kill Klaus right now to save Elena, or will they come up with another way? Whatever they decide, obviously the end result will be they'll have saved Elena. But how will they get there? Who will be on which side when it's over? Who will still be alive?
