Since you have multiple pictures in your gallery and you have a user icon, I'm assuming you want to add an image somewhere where you can't actually add an image... You can't add a picture to the section of your profile with the medals/caps/props/fans tabs. You can only upload pictures to your profile gallery, which you can use for your user icons.
