Previously on Grey's Anatomy, Meredith (Ellen Pompeo) and Derek (Patrick Dempsey) had break up sex, Norman Shales (Edward Herrmann) was the world's oldest intern, Thatcher Grey chose Lexie (Chyler Leigh) over Meredith, Richard's (James Pickens) niece knew she was dying, Alex (Justin Chambers) kissed Izzie (Katherine Heigl) but she pulled away, and George (T.R. Knight) told Callie (Sara Ramirez) that he slept with Izzie.

Tonight's Grey's Anatomy picks up right after George's confession to Callie. She simply says that she forgives him. He made a mistake, but they took vows, so she forgives him.

At work, George frantically looks for Izzie so he can tell her that Callie forgives him. He runs everywhere, ignoring his rounds and running into things, but not fast enough because Callie finds her first, just as Izzie is lecturing her interns pompously that penmanship saves lives. She tells Izzie, “Cafeteria. Noon. You and me. Be there,” which puts the fear of God into Izzie's heart. She thinks that Callie wants to beat the crap out of her, and word gets around to every single member of the hospital staff that there's going to be a fight.

Meanwhile, Derek wants to take Meredith away to wine country for 48 uninterrupted hours of sex. She's in, but she just needs to get someone else to cover her shifts at the hospital, so she makes a deal with Alex. If she can take Norman off his hands, he'll take the shifts. Edward Herrmann is simply adorable as Norman because he tries to buddy up to Meredith by using Grey's Anatomy-isms like “Seriously!” and asking her about McDreamy and McSteamy. Perhaps most awesomely, he greets Meredith's half-sister with a “'Sup, Lexie.” Meredith's job tonight is to teach him how to tell a patient that she's going to die. He doesn't do a very good job of it and the patient, Ms. Bitzer, starts bawling and carrying on. Unfortunately, they discover that he misread the chart and told the wrong patient. But before they can tell her that they made a mistake, Ms. Bitzer has already checked out of the hospital, broken up with her boyfriend, given up her apartment and bought a one-way ticket to Iceland. McSteamy, who is the attending on Ms. Bitzer's case, is very displeased with what's happened, especially because he hates teaching interns. Incidentally, he still considers Meredith to be an intern. In the end, Seattle Grace settles with Ms. Bitzer to prevent a lawsuit, and Meredith takes full responsibility of Norman's mistake.

Cristina, clearly desirous of the title “the new Nazi,” hates interns and continues to address hers by their numbers. Lexie is Three. Lexie tries to stand up to her, refusing to answer to “Three,” but that just serves to piss Cristina off even more. They are both assigned to Derek's service tonight, and their patient is a high school football player who was paralyzed in a game. Now if only Derek had been around when Jason Street got hit, maybe he would still be walking. The boy's father is a crazy football dad and also his coach and berates his son for getting hurt. While they examine the boy, Derek bonds with Lexie about the one thing they have in common, Meredith, a development that doesn't please Meredith when she finds out about it.

The procedure Derek and team decide to do for the paralyzed boy is to drill weights into his head and stretch his body out so that they can fit his spinal cord back in. Or something like that because, as in last week's episode with the needle in the eyeball, I find the act of drilling holes into a person's head, especially while he's still awake, too much to take. While they stretch the boy out, Lexie takes his hand to try to calm him down, which prompts Cristina to yell at her and throw her out of the room. Derek tells Cristina that she is nothing but a big meanie to lil Lexie, and that she better get over herself and teach her how to be a good doctor. So, during surgery, Cristina teaches her how to properly cauterize an artery. Afterwards, Lexie thanks her, and comes to terms with the fact that she will always be Three in Cristina's eyes.

Bailey (Chandra Wilson) has a patient named Ruthie in the clinic whose ankle is swollen and blue from a bad fall on the treadmill. Ruthie is osteoporotic, despite being only 28, and we discover that it's because she's been working out while starving herself in order to lose the 40 pounds that her boyfriend wants her to lose before he can allow her to move in with him. McDoucheBag. Callie tells Ruthie and her boyfriend that she can repair her leg, but she's going to be in a cast for a very, very long time. Longer still if she doesn't start eating normally. The boyfriend is upset that they want Ruthie to gain weight, and begins to argue, but then Ruthie starts to vomit up blood. Tons of it. There's a lot of gross stuff on tonight's episode. Bailey and Callie operate on Ruthie and discover that she has a bleeding duodenal ulcer, and all the ibuprofen she's been taking to relieve her aches from working out too much made it much worse. Callie can't believe that someone would ruin her body like that, but Bailey explains that people are stupid and only want to be loved, and that's they only reason why they do the things they do. And thus, Callie learns her Very Important Life Lesson of the day.

Meanwhile, in chickfight news, Izzie tells Alex that she slept with George and asks him to back her up when she has her cafeteria fight with Callie. In the cafeteria, everyone gathers around Izzie as she prepares for Callie to beat her up. She says that she learned how to fight in a trailer park, and Callie probably learned to fight in boarding school, so Izzie knows that she can hold her own. Callie finally approaches, and Izzie puts up her dukes. This is a most awesome and hilarious scene, but the comic tone instantly changes to one of pathos when Callie realizes that her presence has drawn a crowd. She only wanted to talk to Izzie, not fight, but now that she sees that everyone is staring at her, she just leaves. George, as usual, runs in a hair too late.

Fresh from her humiliation, Callie finds Ruthie's boyfriend outside to tell him that Ruthie died on the operating table. Starving herself for so long made her heart give out. Callie starts yelling at him and saying that he didn't love Ruthie and only stayed with her to feed his ego, but we all know that this is what she wants to say to George. But she can't admit that anything is wrong, not even to Bailey.

Later, Izzie tries to apologize to Callie about the cafeteria incident and about everything with George. Callie almost walks away, but thinks better of it. She lays into Izzie for humiliating her both in her personal life and at work. She ends the tirade with perhaps the best line of the night: “Don't you dare come to me for forgiveness, you traitorous bitch.” Sara Ramirez kind of rocks.

After receiving her dressing-down, Izzie whines to Alex about it, but Alex doesn't want to hear it. He's upset because she had told him that she wasn't ready to be with anyone when he kissed her after Denny died. But then, almost immediately she sleeps with someone Alex despises, and then tells him about it like he's one of her chick friends. Izzies goes home and spends the night crying in bed. This is noteworthy because it was an excuse for Alex to come into her room wearing his black wifebeater. He gives her a box of tissues, and mock-sternly tells her to keep it down.

George finds Callie outside in the pouring rain. He doesn't want her to just say that she forgives when she's still clearly angry. Now, correct me if I'm wrong, but just last week, didn't George say he wanted to divorce Callie? Now he's having a change of heart. She admits that she doesn't forgive him and goes home alone. I wonder if George is going to sleep in the basement room in the hospital where Callie used to live.

While all of this is going on, Richard has some family drama occurring. Adele brings their niece Camille in to the hospital. Incidentally, Camille is not played by Tessa Thompson anymore. She can't breathe, so she must have an emergency tracheotomy done. I couldn't stand to watch that scene either. Camille's cancer has spread all over, including in her throat, but she doesn't want to continue fighting the cancer. She's too tired and wants to spend her remaining days at home with her friends, and not in the hospital. Adele wants Richard to do everything he can to extend Camille's life, but in the end Richard decides to honor his niece's wishes and just let her go home. This doesn't go over well with Adele because this was the one time she asked him to do his job, but he didn't, not even to save his family.

Back to Meredith and Derek. She runs after him and says that she's ready to go away for the weekend, but he cancels the plans. She momentarily gets mad at him for being so chummy with Lexie, but Derek explains that he talks to Lexie about things that he can't talk to Meredith about. Like, about how he wants to marry Meredith, have kids with her, build a house, settle down and grow old in her arms. As usual, Meredith freezes up at this show of feeling, but he understands why she can't talk about it. He says that he knows she's not ready. He says that they can still hook up in the elevator and in the call room until she's ready, but he'll wait for her until she's ready. Derek is looking McDreamier than ever as he says this, but he has to go and ruin it by saying that even though he promises to wait for her, there are no guarantees if someone better happens to come along

Question of the day should he have listened to his ex or honored her wishes?
