During the his fight with mace windu, sidious shot electiricity at windu, but windu deflected it with his lightsaber and it shot back at sidious face, thats how it happened.

yep this person is 100% right his other minor wrinkles were just from old age.
No, he always looked like that. Check episodes 1 and 2. It's just that during his battle against Mace he lost control of the ability to disguise himself. After killing Mace he no longer needed to continue hiding his true nature. Dang.. the Force is cool huh?
