But when they went back to reality she still thought she was stil dreaming and kept trying to kill herself. And she eventually succeeded. He blames himself because he put the idea in her head first.

When they were stuck in limbo (the 4th dreamscape) for years and they thought they would never escape Mal hid the top in a corner of her mind so she could forget that it was a dream. But Cobb found the top and spun it so she would know it was not real and wake up with him by killing herself in the dream. But when she got to the real world she still believed it was a dream and that she had to kill herself so she could "wake up".
