Piece of My Heart:
Addison (Kate Walsh) returns to Seattle Grace to perform an operation and is surprised by all the changes in the lives of her former co-workers; Derek and Meredith's clinical trial has its first patient (Jason O'Mara); Rebecca returns with shocking news for Alex.

The Becoming:
A "date and tell" policy is instated at Seattle Grace after the nurses object to Mark's trysts, Cristina has to face her feelings for Burke and Meredith and Derek admit a gay veteran soldier into their clinical trial.

Losing My Mind:
Dr. Wyatt (Amy Madigan) refuses to let Meredith give up on therapy, Alex learns the truth about Rebecca's medical condition, the Chief insists that Erica perform a risky surgery on his mentor, and after receiving news of Burke, Cristina continues to do things contrary to her nature, going so far as cleaning her always-messy apartment.

Freedom Part one(Finale):
The episode begins with a series of Meredith and Derek's clinical trial, totaling to about 11 since the start of the trial.

Alex takes care of a depressed and confused Rebecca, feeding and bathing her. Izzie urges Alex to get Rebecca professional help, but he insists that Rebecca is fine. He decides to stay home to look after her and gets Izzie to cover all his cases. Alex is preparing food for Rebecca when Izzie calls to check up on them. He turns his back on Rebecca, only to turn around and see her cut her wrists with a knife.

He brings her to the hospital to treat her self-inflicted wounds. Izzie tries to get Rebecca a psychiatric consult but Alex refuses, calling a stunned Izzie “stupid bitch”. She comes back with a psychiatrist and firmly holds her ground. An emotionally charged Alex tries to argue, yelling that he had experienced the same thing with his mother before. After calling Rebecca’s husband, he learns that her husband had left Rebecca two months ago with their baby and that she has underlying Borderline Personality Disorder, something that probably developed after the ferry accident. He eventually decides that sending her to a psychiatric facility would be better for her. She apologizes for not being able to be there for him.

Cristina, Bailey, Erica, Mark and Callie are paged to the ER, where they find a boy almost entirely encased in a block of cement. The patient, Andrew, was dared into lying in a vat of cement, which he did to impress a girl. Bailey comforts the patient by expanding on a Star Wars allusion he made earlier comparing himself to Han Solo being encased in carbonite, telling him that the carbonite was just a piece of the entire picture. Callie tries to convince the girl, Lola, to see Andrew, but she is too shy and cares too much about what her friends would say. Lola ultimately refuses and Callie tells her that she would hate herself for this.

A clot develops in Andrew’s pulmonary artery, but Erica is not around to resolve it. Cristina convinces Bailey and the Chief to let her proceed without waiting for Erica, and they agree. When Erica walks in midway and tries to butt in, Cristina tells her to shut up and the Chief backs Cristina up. After the surgery, the Chief tells Erica that she needs to learn how to teach her residents, an exchange that Cristina overhears. This inspires Cristina to teach Lexie how to perform a running whipstitch. Bailey realizes that her priorities lie with her family and performing surgery, and decides to give the free clinic to Izzie. Callie finds Lola still in the waiting room, and tells her that she should be honest with her feelings. Lola eventually goes to Andrew’s bedside and kisses him.

George continues to work hard as the Chief’s Intern to impress the Chief in the hopes that he can become a resident. As he helps the Chief research on cement toxicology, he stumbles upon the files on all the residents in the hospital. After being taken off the cement surgery, he vents to Lexie that although he has so much power as the Chief’s intern, he does not get to do anything because he is still an intern. Overcome by temptation, she sneaks into the Chief’s office and steals the residents’ files. She tells George that he failed his intern test by one point (even though it was not a pass/fail exam but rather an exam where the lowest 5 scoring interns would be cut[1] ). He is angry at her for letting him know that he just almost made it. She also lets him know that he had the highest number of recommendations, which spoke of how great a doctor he was. George later tells the Chief he deserves a second chance, and the Chief agrees to let him retake the intern exam.

Meanwhile, a young couple, Beth and Jeremy, are admitted into the clinical trial. However, the Chief tells Meredith that after losing 11 patients, the Institutional Review Board will shut down the clinical trial at midnight if they lose a twelfth patient. Meredith convinces Derek to go around the rules by operating on both Beth and Jeremy before midnight, and promises to clear it with the Chief first. Beth confesses to Meredith and Derek that she and Jeremy are very much in love and want to consummate their relationship. Meredith and Derek make arrangements to give them some time alone. Afterwards, Jeremy is taken into surgery but dies on the table.

After breaking the news to Beth, Derek dumps the celebratory champagne he bought for when the clinical trial first succeeds. He tells Beth that she does not have to go through with the surgery, but she insists on proceeding because Jeremy would not want her to give up. Derek is furious at Meredith for encouraging Beth to participate in a doomed procedure, and after Beth’s surgery, wants nothing more to do with her. Beth survives the procedure, and Meredith discovers that her tumor is shrinking. She asks Rose to tell Derek the good news, but Rose tells her that she should do it herself. Meredith frantically searches for Derek and the bottle of champagne. Meanwhile, Derek sees that Beth is conscious and better, and grabs the champagne from the trash can.

Meredith confesses to Dr. Wyatt that when she was little, her mother slashed her wrists with a scalpel after Chief Webber left her, telling Meredith to be "extraordinary". Dr. Wyatt tells Meredith that she is angry at the Chief, her mother and herself. The Chief confronts Meredith about cramming both surgeries before midnight, but she turns it on him by telling him about her mother’s suicide attempt. With Dr. Wyatt’s help, she realizes her mother was just trying to get Chief Webber’s attention by slitting her wrists. If her gifted surgeon of a mother really wanted to kill herself, she would have cut her carotid artery instead, which would have been instantly fatal. Impressed, Dr. Wyatt tells Meredith she is just like her mother except that now she can learn from her mother’s mistakes.

Mark encourages Callie to overcome her fear and approach Erica, saying that he is growing. Callie goes up to Erica and kisses her. Chief Webber goes back home to Adele and tells her that he is her husband and he wants to come home to his wife. Adele replies saying that that it was about time, and they kiss. George tells Lexie that the Chief is letting him retake the intern exam, and gives her a platonic kiss on the mouth, causing Lexie to touch her lips as if to contemplate if she has feelings for George. Izzie apologizes to a devastated Alex for Rebecca and his mother, and he kisses her. She pulls away, but he desperately begs her to let him. Alex breaks down and cries on her shoulder.

Derek eventually finds Meredith on his land where he planned to build their future house, muttering to herself about how stupid and cheesily corny what she's doing is. She had outlined their future house with candles, and yells at him for taking so long to show up when she is now “whole and healed”. He interrupts her mid-rant by kissing her, and pulls away. He tells her to stay where she is and wait for him, while he goes to talk to Rose.

Season Five
Dream a Little Dream of Me, Part 1:
A military doctor piques Cristina's interest -- and she helps him -- and he helps her when she gets stabbed by an icicle. Meredith and Derek attempt to work on their "happily ever after", and the attendings are stressed over Seattle Grace's ranking of #12.

Dream a Little Dream of Me, Part 2:
Alex is offended when Izzie talks to Meredith about how great of a man Alex is. Derek is stabbed with the scalpel in the palm of his hand by Rose, while the Chief is stunned over Mercy West's increase in the number of patients, a deadly ice storm ravages Seattle.

Here Comes the Flood:
A plumbing leak causes a flood at Seattle Grace, as the Chief tries to implement new teaching policies and George retakes his intern exam. Derek tries to move Izzie and Alex out of Meredith's house.

Brave New World:
Meredith freaks out when Derek discovers her mother's old diary in the house, Callie frets as she prepares for her first official date with Erica, and Cristina stumbles into a part of the hospital that neither she nor her fellow residents have seen before.
There's No 'I' in Team:
Bailey heads up a team of surgeons performing a "domino procedure" in which each surgery hinges on the one preceding it. Derek gets sole credit for the brain tumor study he did with Meredith. Lexie goes to great lengths to have George notice her romantically, and Sloan counsels a desperate Callie on the ways of love.

Life During Wartime:
To help Bailey become the best general surgeon at Seattle Grace, the Chief gives her new power, new responsibility, and the challenge of removing a young girl's inoperable tumor. The new head of trauma, Owen Hunt, challenges both the attending doctor's methods and the residents' surgical abilities.

Rise Up:
The Chief announces to the residents that a solo surgery will be awarded, and the news sends them into a competitive frenzy. Meanwhile Derek turns to Bailey for advice when he feels Cristina is interfering in his relationship with Meredith, and another dark day in Izzie's career rises when Erica discovers a shocking connection between her and a longtime patient.

These Ties That Bind:
One of Meredith's oldest friends, Sadie (Melissa George), becomes an intern at Seattle Grace, but Mer's friends are less than welcoming to her. Lexie leads her fellow interns in secretive, unorthodox surgical training sessions; and a new surgeon, Virginia Dixon (Mary McDonnell), brings rules and order to the OR.

In the Midnight Hour:
Meredith, Cristina and Bailey come to Lexie and Sadie's rescue when a not-so-routine surgery goes horribly wrong, as Owen and Derek treat a man seriously injured while sleepwalking and Mark comforts the sleepwalker's distraught daughter and has feelings for Lexie.

All By Myself:
Cristina is awarded the first solo surgery among the residents, but she has to relinquish it and choose her replacement; Mark performs a cutting-edge procedure to restore a patient's speech, impressing Lexie in the process; and Callie cannot figure out if Sadie (Melissa George) is flirting with her or just being friendly.

Wish You Were Here:
Bailey teams with Seattle Grace's new pediatric surgeon, Dr. Arizona Robbins, to save the life of a young patient near and dear to her heart, as the admittance of a death row inmate (Eric Stoltz) at the hospital stirs anger in Derek and further erodes Meredith and Cristina's friendship; meanwhile, Callie and Sloan work as a team to fight their respective attractions to Sadie and Lexie, and Izzie tells Alex that she is seeing Denny.

Sympathy for the Devil:
Derek's mother, Carolyn (Tyne Daly), makes a surprise visit to Seattle and meets Meredith for the first time, as Mark tries to conceal his relationship with Lexie from her as well; meanwhile, death row inmate William (Eric Stoltz) wishes to donate his organs to Bailey and Arizona's patient, as Owen and Cristina try to go on their first date.

Stairway to Heaven:
Bailey grows desperate as a patient's condition becomes more and more dire. Everyone in the hospital is stepping up to try to find the boy a new organ donor, including Meredith, who considers a death row patient's wish to die before execution. Meanwhile, an injury leaves Mark in a compromising position in the on-call room and Cristina awkwardly avoids Owen. Izzie is still struggling with Denny as he is constantly reminding her "He is there for her". Izzie has an emotional blow out with Denny and soon understands what he is trying to tell her. She finally comes to the realization that Denny is not here "for her" in the emotional sense, which leaves Izzie reeling when she soon discovers that she is sick. Jeffrey Dean Morgan Guest stars.

Beat Your Heart Out:
Now that Derek has the engagement ring, he searches for the perfect way to propose to Meredith, as Lexie demands that Mark go public with their relationship, and Bailey, encouraged by her work on recent cases and by Dr. Dixon (Mary McDonnell), considers a new direction for her medical career. Dr. Dixon suggests that Dr. Bailey will make a good pediatric surgeon. As Derek finally decides to propose to Meredith, Addison calls him, and he decides to postpone the proposal. Cross-over with Private Practice

Before and After:
Addison (Kate Walsh), Naomi (Audra McDonald) and Sam (Taye Diggs) rush the ailing Archer to Seattle Grace and reunite with old friends Derek and Mark to help save his life; meanwhile, Izzie employs her fellow doctors in a medical game designed to help teach the interns, and Owen runs into a significant someone from his past. Cross-over with Private Practice.

An Honest Mistake:
Derek's confidence is shaken like never before as he faces off with Addison (Kate Walsh) over what course is best for their dying pregnant patient; Cristina calls the abilities of a renowned surgeon (Faye Dunaway) into question after she botches a standard procedure; and Bailey, who has decided to pursue a fellowship in Pediatric Surgery, is appalled by the Chief's less-than-stellar recommendation letter. Cross-over with Private Practice

I Will Follow You Into the Dark:
Izzie walks the interns through diagnosing Patient X after receiving her troubling test results. Bailey and Meredith perform a surgery where they remove a woman's stomach to protect her from a very invasive form of stomach cancer that runs in her family. Hunt and Karev treat a high school marching band player with seizures. Shepherd has to come to grips with killing one of his patients. Cristina receives some disturbing news from one of her colleagues.

Stand By Me:
After his fellow surgeons try and fail to bring Derek back to work, Meredith must summon the strength to convince him. Meanwhile the hospital is abuzz as Mark attempts a rare face transplant surgery. Cristina, at last awarded her first solo surgery, is more focused on the needs of her friends, and Izzie does her best to keep her medical condition a secret.

Elevator Love Letter:
All eyes are on Derek as he is set to perform his first surgery after his breakdown; attention is also set on Izzie, whose medical condition requires her to undergo a difficult surgery; and Owen has another PTSD episode that injures Cristina and traumatizes both of them, throwing the future of their relationship into question. Derek proposes to Meredith in the elevator.

Sweet Surrender:
As she endures aggressive treatment for her cancer, Izzie throws herself into planning Derek and Meredith's wedding, Callie introduces Arizona as her girlfriend to her disapproving father (Hector Elizondo), George excels in a trauma emergency when Alex freezes, and Owen seeks help with his PTSD from Dr. Wyatt (Amy Madigan). Meanwhile, Bailey learns of the heart-wrenching practices in peds.

No Good at Saying Sorry:
As preparations continue for Meredith and Derek's wedding, Izzie receives a surprise hospital visit from her handful-of-a-mother, Robbie. Meanwhile, a newly sober Thatcher visits his daughters, Meredith and Lexie, in order to make amends. The Chief and Meredith bitterly fight over a controversial medical case.

What a Difference a Day Makes:
As Meredith, Derek and all of their friends prepare for the big wedding, a group of college students, injured in a car crash as they were heading to their graduation ceremony, is admitted to the ER. Meanwhile, Izzie fears the worst when she begins experiencing familiar symptoms. It turns out she has another brain tumor, which makes Derek and Meredith give their wedding to Izzie and Alex who get married instead.

Here's to Future Days:
Izzie spends time with a fellow cancer patient and contemplates whether or not to undergo a risky surgery, as Mark is ready to take his relationship with Lexie to the next level and a patient makes Owen reassess his place at Seattle Grace. Izzie's surgery to remove the brain tumor could have dire consequences.

Now or Never:
George delivers stunning news to Bailey, sending shock-waves throughout the hospital, and Izzie's friends await her recovery from surgery. Meanwhile Bailey is surprisingly displeased after being accepted into the pediatric fellowship program. The doctors focus on the victim of a near-fatal traffic accident who is disfigured and unable to speak. Meredith discovers it's George, who was hit by a bus while saving the woman who accompanied him to the hospital. Izzie recovers her memory and tells Alex that she loves him before losing consciousness. The episode ends with both George and Izzie flatlining and meeting up with each other in a heaven-like scene, with no definite indication of whether either or both are dead or alive

Season Six:

Good Mourning:
Izzie has a bad wake-up call, George's death. Meredith and the other residents fear to continue with their lives as George's death is a major impact to them all.

Goodbye:
Cristina and Owen are finding it difficult to abstain from sex as instructed by Dr. Wyatt. Meredith and Derek consumate their wedding everywhere they go. Izzie mourns her best friend, George, after his death. Richard announces Seattle Grace will be merging with Mercy West.

I Always Feel Like Somebody's Watchin' Me:
Bailey's and Alex's case is complicated when the patient's schizophrenic son arrives. The residents and interns of Seattle Grace compete to survive the budget cuts.

Tainted Obligation:
When Meredith's and Lexie's father, Thatcher, returns to the hospital with a failed liver, it's up to Meredith to save his life. Meanwhile, Izzie empathizes with a patient riddled with tumors, as Mark, annoyed with Cristina's competitive zeal, tricks her into assisting on an unusual surgery.

Invasion:
The residents of Seattle Grace and Mercy West vie for surgeries and their careers as day one of the hospital merger begins. Meanwhile, Arizona stands by an enraged Callie when her father visits with a priest to attempt to convince her to renounce her homosexuality; Izzie experiences the brunt of the new surgical competition; and an accident derails a patient's upcoming kidney donation, with Dr. Bailey promising to set things right, segueing into a crossover with Private Practice. Izzie is fired and leaves a Dear John letter for Alex and disappears.

I Saw What I Saw:
After a burn victim unexpectedly dies in the midst of a chaotic ER, the Chief and board member Jennings interrogate the residents to find the culprit, as all the Mercy West and Seattle Grace residents defend their actions in an effort to save their jobs. Dr. April Kepner (Sarah Drew) is fired because she forgot to look inside that burn patient's throat, a mistake that was the main cause of the patient's death.

Give Peace A Chance:
Derek must help the unlikeliest of all; the hospital lab-tech, Isaac (Faran Tahir), when it turns out that he has an inoperable tumor surrounding his spine. Derek is juggling two things at once when he questions Richard's authority.

Invest In Love:
Arizona is stunned when her longtime patient's parents offer the hospital a donation of twenty-five million dollars, but when her patient's condition worsens, all eyes are on her to save him. Cristina tests her relationship when she blatantly disregards Owen's orders and performs an unauthorised cardiac procedure, then later kisses Mercy Wester Jackson Avery at a surprise party for Arizona. Alex cares for a struggling newborn while Izzie remains missing. In the wake of great grief, Arizona and Callie take their relationship to the next level.

New History:
When Owen recruits his fellow Iraqi War vet, Dr. Teddy Altman (Kim Raver), as the new cardiothoracic surgeon, Cristina questions Teddy's abilities in the OR and the nature of their past relationship. Meanwhile Izzie returns to Seattle Grace with her high school mentor (Joel Grey) seeking treatment for his dementia, and the stress of Richard's responsibilities, post-merger, are taking their toll.

Holidaze:
As Thanksgiving, Christmas and New Year's pass, Miranda is visited by her father, William, who disapproves of her choices in life. Meanwhile, Mark and Lexie must cope with the shocking arrival of a woman from his past, his "daughter". Thatcher Grey questions the Chief's recent behavior as Meredith comes to his defense. Cristina suspects that there is/was something going on between Owen and Teddy.

Blink:
Part 1 of a Grey's Anatomy/Private Practice crossover. Mark flies Addison (Kate Walsh) to Seattle to help with a difficult procedure on his pregnant daughter, Sloan,which creates tensions between him and Lexie leading to their break-up. After learning of Richard's drinking problem, Derek confronts him for the sake of his friend and the hospital, Izzie returns, hoping to reconcile with Alex, and tensions run high between Teddy, Cristina and Owen in the aftermath of Teddy's startling confession.

State of Love and Trust:
As Derek begins his role as interim chief, he faces a potential lawsuit when Bailey and Meredith's patient awakens from anesthesia mid-surgery. Meanwhile, Teddy refuses to place Cristina on her service, as Arizona tests Alex out in Pediatrics, and Mark refuses to speak to a heartbroken Lexie. Derek tries to convince Richard to seek treatment for his drinking.

Valentine's Day Massacre:
It's Valentine's Day, and the doctors must treat dozens of injured people after the roof collapses at a popular romantic restaurant; Derek tries to manage his busy schedule as Chief of Surgery; Meredith wrestles with her new duties as the Chief's wife; and Mark and Callie team up to convince Sloan to do the right thing for her baby.

Time Warp:
In his new role as Chief, Derek restores the hospital lecture series, and Richard, Bailey and Callie present to the group pivotal surgical cases from their pasts - Bailey reflects on her first days as a shy intern, Callie looks back on a polio case, and Richard recalls a case in which he and Ellis worked on a patient diagnosed with GRID, a disease later called AIDS. The flashback-heavy hour sheds light on the beginnings of the affair between the Chief (played by J. August Richards) and Ellis (Sarah Paulson) in 1982. In separate 2004 sequences, viewers see Bailey in her pre-”Nazi” days and Callie shortly before she was introduced on the show

Perfect Little Accident:
When famous surgeon Dr. Harper Avery is rushed to Seattle Grace-Mercy West, the staff is surprised to discover that one of their own is related to this legend, and his unorthodox request for his surgery causes tensions between Derek and Richard. Meanwhile Callie and Arizona try to help Teddy and Mark move on from their pasts and find greener pastures.

Push:
A patient who needs an extremely difficult surgery comes to Seattle Grace, and it's Richard and Owen who vie for the opportunity to tackle it. But the decision lies in the hands of Derek, who deems that Owen should take on the surgery, leaving Richard bitter. Meanwhile, Bailey deals with her own personal struggles when she nervously prepares for another date with Ben, and Mark and Teddy go out on a strange date.

Suicide is Painless:
When Teddy's patient (Sara Gilbert) seeks to end treatment and her life, she turns to Hunt for help. The situation triggers Hunt's memories of his life at war and his past relationship with Teddy. Meanwhile, Callie and Arizona have a difference in opinion on what their future holds, Richard tries to adjust to his role as a surgeon, and Meredith becomes angry at Derek when she feels he stole one of her surgeries.

Sympathy for the Parents:
When Alex's younger brother, Aaron, shows up at Seattle Grace-Mercy West with a hernia, it's up to Alex to get Bailey's approval for his pro bono surgery. Matters are further complicated when Aaron discloses that his family has not heard from Alex since he arrived in Seattle, and parts of Alex's past long kept secret are revealed to his friends and fellow doctors. Meredith and Derek discuss children. Teddy and Mark have sex for the first time.

Hook, Line and Sinner:
Sloan returns to Mark’s apartment, just as she’s going into labor, and Teddy, Mark and the still-fighting Callie and Arizona are there to help with the birth and to get Mark through his grandson’s pending adoption. Meanwhile Teddy sees Derek’s invitation to illustrious cardiothoracic surgeon Tom Evans as a threat to her future employment at the hospital, and the team works on a crab boat captain who has been stabbed with a giant shark hook.

How Insensitive:
Bailey preps the team with mandatory sensitivity training prior to admitting a 700-pound patient with compounded medical issues, and the case proves to be challenging in every sense of the word. Meanwhile Derek has to come face to face with a former patient’s husband in a wrongful death deposition, and spending time with a heart patient’s daughter opens up some old wounds for Cristina.

Shiny Happy People:
An elderly patient admitted into the E.R. for a heart condition sees a familiar face, a long lost love (guest star Marion Ross) who happens to be in the E.R. as well for a fractured arm, and the staff find themselves caught up in their love story. Meanwhile, Karev treats a troubled teenage patient Hayley (guest star Demi Lovato), whose parents brought her in for schizophrenia, and Meredith can't help but tell Cristina about her suspicions of Owen -- which inevitably messes with Cristina's head.

Sanctuary:
Part one of the two-hour season finale. Gary Clark enters Seattle Grace Hospital with a gun to seek revenge for his wife's death. He kills Reed with a single bullet to the head, shoots Alex and then leaves to find Derek, who he holds responsible for his wife's death. The hospital goes on lockdown and a pregnant Meredith and Cristina are stuck in a supply closet waiting for all of it to be over. Mark and Lexie find Alex heavily bleeding on the floor of the elevator and try to fix him and stop the bleeding. Bailey is with Dr. Percy and a patient named Mary (guest star Mandy Moore) and are soon found by Gary. Mary plays dead, Bailey hides under the bed, but Dr. Percy is found and shot. Callie and Arizona are stuck on the Pediatric section along with the young children. Teddy, Owen and Jackson are operating on a patient, but are not allowed to leave to the ICU afterward. Cristina discovers Gary's after Derek and she tells Meredith, who runs out of the room to find Derek. Gary finds him first and shoots Derek.

Death and All His Friends:
Part two of the two-hour season finale. Meredith and Cristina wheel Derek to the OR to find Teddy, but she and Owen have already left with their patient, so Cristina and Jackson decide to do the surgery instead. Mark and Lexie have stopped the bleeding in Alex's chest; while delirious, Alex calls out for Izzie, begging her to come back. Richard, who is outside the hospital and hears all the emergency calls from the doctors who are inside, goes inside the hospital. Worried about Cristina, Owen goes back inside too, finally choosing between her and Teddy. Owen finds Cristina and sees Gary who, unbeknownst to Meredith, has taken the OR hostage. Owen gets shot while trying to overpower Gary and Cristina and Jackson let Gary believe that Derek is dead so that he will leave. Once Gary is gone, they continue the surgery and Meredith takes care of Owen, while miscarrying right in front of him. Dr. Bailey is unable to operate on Dr. Percy and he dies in Bailey's arms. Richard finds Gary, who shoots himself. Derek and Owen survive; Alex is taken away by an ambulance. Callie and Arizona reunite and decide to have a baby. The hospital turns into a crime scene and Meredith silently mourns the death of her unborn baby.
